Pre-Lesson Covid-19 Screening Form

Although we have taken every precaution to ensure compliance with Covid-19 regulations, and to ensure the safety of our students and instructors to the very best of our ability, we are required to have all students (or their parents/guardians) complete this Screening Form prior to their lesson in the unlikely event that contact tracing needs to take place at any stage.
